Elderflower has two membership options to meet patient needs. Both memberships offer the same primary care services, but vary by location of services. The practice is flexible and can change memberships based on patient needs.

Elderflower Office Visits

Elderflower Home Visits

  • One time enrollement fee of $150.

    • Vitals

    • Ear Wax Removal

    • Medication Ordering/ Refills

    • Lab Ordering

    • Referrals

Clinic visits can be disorienting and increase agitation and discomfort in patients, especially those with dementia. I come to patients where they are most comfortable and least disruptive to their lives.

Procedures Not Included

This practice is not a med spa and will not perform botox injections, hormone pellets, or IV vitamin infusions.
